QWKPoll appears to have imported 1756 messages, with 16 failures - not sure why 16 failed (no info in log file).
Hmm, never seen a failure with that I think -- but maybe it's just messages that it considers dupes for some reason, and they ended up in the dupe
message base?

The message appears to be missing a tearline and (addressless) origin line.
Do you have "Base Type: Networked" set for the QWK message bases?
Here's a short excerpt of my settings for this area:
Name ³ 2010: DOVE-Net Sysops Only
Base Type ³ Networked
Net Address ³ 0:0/0 (Local Address)
Export To ³ 0 node(s)
Origin ³ <empty; it will use the default Origin then>
QWK Network ³ DOVE-Net

Thats the "max" settings I could select that I believe work fine (at least so far).
